Yes, Plasencia Family Medicine is currently accepting new patients of all ages in Saginaw and throughout Mid-Michigan, including Bay City, Midland, Freeland, Frankenmuth, Bridgeport, and surrounding communities. Yes, we are in-network with Blue Cross Blue Shield of Michigan (BCBSM), as well as Medicare and most major commercial insurance plans. If you are unsure whether your specific plan is accepted, please call our office and our team will be happy to verify your coverage before your first visit.

Yes, we offer same-day sick visits for established patients who are experiencing acute illness or new symptoms. Same-day availability depends on the time of day and clinical urgency, so we recommend calling our office first thing in the morning when possible.

We make transferring care simple. Our practice uses MyChart, a secure online patient portal that allows you to message your care team, request prescription refills, view test results, and access your health information from any device. To transfer records from a previous provider, simply sign a medical records release form at your new patient visit and our team will request your records on your behalf. Bringing a current medication list and any recent lab or imaging results to your first appointment helps us get to know you faster and provide more informed care from day one.

Please bring a valid photo ID, your current insurance card, a list of your current medications and dosages, and any recent lab results, imaging reports, or specialist notes if available. New patient intake forms can be completed online or in our office before your appointment.

Plasencia Family Medicine is located at 4705 Towne Centre, Suite 202 (Medical Arts 1 Building) in Saginaw, Michigan. We are open Monday through Thursday from 8:20 AM to 5:00 PM and Friday mornings from 8:20 AM to 12:00 PM.
