Our practice was founded by Dr. Jorge M. Plasencia, MD FAAFP, whose commitment to compassionate, patient-centered care has made him a trusted name in Mid-Michigan for decades. Dr. Ileana M. Daly, MD — Dr. Plasencia's daughter — is proud to carry that legacy forward, bringing the same warmth and dedication to a new generation of patients and families.
Our care team is strengthened by Courtney and Karissa, who have each been an important part of our practice for over 10 years. Their experience, kindness, and familiar friendly faces help create the welcoming environment our patients know and appreciate. From greeting patients with a smile to helping keep visits running smoothly, Courtney and Karissa play an essential role in making every patient feel seen, supported, and cared for.

Jorge M. Plasencia, M.D. FAAFP
Some doctors treat conditions. Dr. Jorge M. Plasencia treats people. For more than two decades, that distinction has defined his practice — and it is why patients who found him twenty years ago are still calling his office today.
Dr. Plasencia earned his medical degree from the University of Costa Rica (UCR) and completed his residency training at Saginaw Cooperative Hospitals (now Central Michigan University) in Saginaw, Michigan — a city he has called home ever since. In 2005, he founded Plasencia Family Medicine with a single, clear vision: to build a practice where patients are never rushed, never reduced to a chart number, and always leave feeling genuinely cared for.
Over more than 25 years of practice, Dr. Plasencia has become one of the most trusted names in Mid-Michigan medicine. Patients consistently describe him the same way — he listens, he asks the right questions, he remembers who you are, and he treats you like family. That reputation was not built overnight. It was earned one appointment at a time, one patient at a time, across thousands of families throughout Saginaw, Bay City and the surrounding communities.
Board certified by the American Board of Family Medicine and a Fellow of the American Academy of Family Physicians (FAAFP), Dr. Plasencia brings both the credentials and the experience that patients deserve in a primary care physician. His deep roots in the Latino community have also made him a vital resource for Saginaw's Spanish-speaking patients, many of whom have followed him for decades and trust him with the health of their entire families.
Those who know Dr. Plasencia well know that his approach to medicine is deeply personal. He has long believed that the relationship between a doctor and a patient is one of the most sacred in human life — built on honesty, consistency, and genuine concern. It is a belief he has passed on, through example and mentorship, to his daughter Dr. Ileana Daly, who now practices alongside him and is carrying that legacy into the next generation of care.
Dr. Plasencia has no plans to slow down. If anything, the arrival of his daughter as a fellow physician has reinvigorated his commitment to the practice and the patients he has served for so long. He is as dedicated today as he was on the first day he opened these doors — and he intends to keep it that way.
Outside of medicine, Dr. Plasencia is a proud husband, father, and grandfather. His family is his greatest source of joy and the quiet motivation behind everything he does. He finds in his grandchildren the same sense of wonder and possibility that has always driven him to show up for his patients with full presence and full heart.
Ileana M. Daly, M.D.
Growing up in Saginaw, Dr. Ileana M. Daly learned what it meant to be a truly great doctor long before she ever set foot in a medical school. She watched her father, Dr. Jorge M. Plasencia, show up for his patients day after day — with patience, with warmth, and with the kind of genuine care that made people feel like they were his only patient. It shaped everything about the physician she chose to become.
Dr. Daly earned her medical degree from the University of Medical Sciences (UCIMED) in Costa Rica, an experience that deepened both her clinical foundation and her ability to connect with patients across cultures and languages. She returned home to Michigan to complete her Family Medicine Residency at Central Michigan University in Saginaw — a deliberate choice, rooted in her love for this community and her desire to build her career here.
Today, Dr. Daly practices the same kind of medicine she grew up watching — unhurried, personal, and deeply attentive. Her clinical interests span preventive medicine and lifestyle counseling, women's health and menopause care, medical weight management including GLP-1 therapy, chronic disease management, and mental health support. She believes that the best medicine treats the whole person, not just the condition — and that building a relationship with your doctor over time is one of the most powerful investments you can make in your health.
One of Dr. Daly's greatest passions is helping women navigate the often confusing and underserved transitions of perimenopause and menopause — a stage of life she believes deserves the same medical attention and respect as any other. She is equally committed to helping patients achieve sustainable weight loss through evidence-based approaches, combining the latest medical therapies with honest conversations about nutrition, movement, and long-term lifestyle change.
As a bilingual physician fluent in both English and Spanish, Dr. Daly is proud to serve Saginaw's Spanish-speaking community — bringing the same standard of personalized, compassionate care to patients who deserve a doctor who truly understands them, in every sense of the word.
Outside the office, Dr. Daly is a proud wife and mother of two. Her husband is a product designer, and together they've built a home full of creativity, curiosity — and the inevitable chaos that comes with children and their beloved Yorkie. She finds that being a parent has made her a better physician: more empathetic, more attuned to the real pressures families face, and more committed than ever to meeting patients exactly where they are.
She is honored to carry her father's legacy forward — and to be your family's doctor for the years ahead.
Meet Our Friendly Staff
Courtney Musinski, MA
Medical AssistantCourtney is a Bay City native and a proud, long-standing member of the Plasencia Family Medicine team, celebrating 15 years with the practice in May 2026. After completing her college education, she gained early clinical experience at another physician’s office before joining the practice — and she hasn’t looked back since. Her dedication to the patients and the team she serves is evident in everything she does.
Outside of work, Courtney’s world revolves around her two kids. Her son Gibsonn keeps the family calendar full year-round with travel soccer, football, and basketball, while her daughter Sylvie brings equal energy to dance, basketball, and flag football. Busy doesn’t begin to cover it!
When she does get a moment to herself, Courtney is most at home on the water — boating, kayaking, or paddleboarding whenever she can. She is a true Michigan summer enthusiast who loves heading up north, camping, and spending time at the beach. Michigan’s outdoor seasons were practically made for her.
Courtney also has a passion for travel and discovering new places. Her favorite destination is Key West — she is drawn to the laid-back island culture and vibrant community there. Closer to home, you’ll find her biking, tending to her garden, and enthusiastically cheering on her beloved University of Michigan Wolverines. Go Blue!
Karissa Sharrard, MA
Medical AssistantKarissa has been part of our office for over 13 years and truly loves what she does. She is passionate about helping patients feel comfortable, cared for, and supported every step of the way. Karissa cannot imagine herself outside of the medical field, where she continues to find purpose and fulfillment in helping others every day. She is very proud to be part of this team and values the relationships built with both patients and coworkers.
Karissa stays busy with her son, Dominick, who plays football year-round—from flag and tackle to camps with players from the Detroit Lions and the University of Michigan. Her family loves football, cheering “Go Blue” and “Go Lions” (with a little “Go Packers” for her fiancé).
When she is not at work, at a game, or wedding planning, Karissa enjoys spending time with family and friends and making the most of Michigan’s outdoor lifestyle. She loves attending concerts—especially 80’s music—along with camping, tubing, kayaking, and enjoying time on the Great Lakes whenever she can. Born and raised in Michigan, she has a strong appreciation for everything the state has to offer and looks forward to continuing to build her life here. She’s proud to call Michigan home.
