Step 2 — Complete your new patient forms.
Complete your new patient paperwork before your visit. Download them here. Having these ready in advance means more time with your doctor and less time in the waiting room.

Step 3 — Come meet us.
Arrive a few minutes early for your first visit. Our team will welcome you, verify your information, and get you settled in. Then you'll have dedicated time with Dr. Daly or Dr. Plasencia — no rushing, no shortcuts. Just real medicine and real conversation.

What to bring to your first visit.
To make your first appointment as smooth as possible, please bring the following:
Photo ID (driver's license)
Insurance card(s)
A list of all current medications and dosages
Any relevant medical records, lab results, or imaging from previous providers
A list of current health concerns or questions you'd like to discuss
Names of any specialists you currently see
Please plan to arrive 10–15 minutes before your scheduled appointment time for your first visit. This allows our team to verify your information and get you checked in comfortably.

We currently schedule new patient appointments by phone. Call us at (989) 791-3401 during office hours (Monday–Thursday, 8:00 AM – 5:00 PM) or Friday 8:00-12:00pm. Our team will find a time that works for you and answer any questions you have before your visit.

You will be scheduled with either Dr. Ileana M. Daly, MD or Dr. Jorge M. Plasencia, MD FAAFP — both board-certified family physicians. Every appointment is with one of our physicians directly. If you have a preference, just let us know when you call.

Yes — we offer same-day and next-day appointments for acute illness and urgent concerns when availability allows. Call us first thing in the morning for the best chance of a same-day slot at (989) 791-3401.

Yes — we offer telemedicine visits for a wide range of concerns including follow-up appointments, medication management, and acute illness. Ask about telemedicine availability when you call to schedule.

Yes — we provide care for patients of all ages, from newborns to seniors. Plasencia Family Medicine is a true family practice, and we welcome the whole family under one roof.
